To register online for the Second Biennial Meeting & Reunion on July 20-22, 2018 in New York City, please follow the following 2 steps:

- Pay either by Credit/Debit Card through PayPal or by sending your check (see below, after the Form).
Please be reminded of the deadlines outlined in the figure. If you are bringing guests, please indicate appropriately. Guests for Friday’s Welcome Reception are charged $65 per person, while guests for Saturday’s Dinner Banquet are charged $75 per person.

Paying by Check
If paying by check, please write a check for the Total Payment indicated above payable to USAPMTIAA. Your registration will be processed once check is received. Please mail the check to:
