To register online for the Third Biennial Meeting & Reunion on June 12-14 , 2020 in Orlando, Florida , please follow the following 2 steps:

Pay either by Credit/Debit Card through PayPal or by sending your check (see below, after the Form).

Please be reminded of the deadlines outlined in the figure. If you are bringing guests, please indicate appropriately. Guests for Friday’s Welcome Reception are charged $65 per person, while guests for Saturday’s Dinner Banquet are charged $75 per person.

PLEASE NOTE: For those bringing guests to the Welcome Reception, please list down the name and age of each guest. If not bringing one, please type in “NA” for not applicable. Note: Children 10 years old and below have 50% discount ($32.50). Ages 11 and up are full price of $65. Please make the necessary adjustment when you enter your Total Payment above.

For those bringing guests to the Saturday Dinner Banquet, please list down the name and age of each guest. If not bringing one, please type in “NA” for not applicable. Note: Children 10 years old and below have 50% discount ($37.50). Ages 11 and up are full price of $75. Please make the necessary adjustment when you enter your Total Payment above.

Paying by Credit or Debit Card

Paying by Check

If paying by check, please write a check for the Total Payment indicated above payable to USAPMTIAA. Your registration will be processed once check is received. Please mail the check to:
