Membership to the USAPMTIAA requires submission of completely filled registration form and payment of the annual membership due of $25 payable to USAPMTIAA. You can choose from either of the following ways to do this.

Applying by Mail
Please download and fill up the membership form by clicking the Membership Registration Form. Mail this form with your check to our Treasurer in the address below. Payments by check or by credit cards (through PayPal) are accepted. Please write check payable to USAPMTIAA.

Do you know that you can apply for Lifetime Membership and will not need to pay Annual Membership Fee? If you donate at least $300 to the Alumni Endowment Fund, you automatically become a Lifetime Member and you are exempt from paying annual dues. Please contact our Treasurer (Dr. Marlon Torrento) if you want to be a Lifetime Member and Donor to our Alumni Endowment Fund.
